About the Job

PATH is seeking a skilled Case Manager to join our Housing team in Santa Barbara, CA. As a key member of our team, you will provide field-based direct client outreach, linkage, and retention services to veterans experiencing homelessness.

Key Responsibilities:

  • Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans to address clients' needs.
  • Collaborate with clients to set and achieve short- and long-term goals, providing ongoing case management support to ensure treatment plan outcomes are met.
  • Provide field-based therapeutic supportive case management services to address mental health, functional, and substance use barriers, helping clients obtain and maintain housing.
  • Consult with LCSW and supervisor to conduct crisis and suicide risk assessments and provide appropriate interventions.

Requirements:

  • Master's Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences.
  • One year of experience in healthcare, case management, homeless services, or working with vulnerable populations.
  • Valid driver's license, reliable transportation, and proof of insurance and ownership for personal vehicles used during work duties.
