Hospice Chaplain

To provide and coordinate spiritual care services for hospice patients and their families, ensuring compliance with Medicare Hospice Conditions of Participation and industry standards.
Key Responsibilities- Provide spiritual support and counseling services through home visits and individual/group counseling sessions.
- Coordinate spiritual care for terminally ill patients and their families, utilizing personal, community, and volunteer resources.
- Assess and document spiritual needs, issues, and concerns of hospice patients and families.
- Develop and implement training and education programs for staff and volunteers on spiritual care and support.
- Oversee the development and coordination of spiritual care resources, including literature, support groups, and community resources.
- Measure and monitor the quality and quantity of pastoral care and spiritual support services provided.
- Provide bereavement support to surviving family members for up to 13 months after patient death.
- Minimum 2 years of experience providing spiritual care services in a healthcare setting.
- Minimum 1 year of experience providing services to the terminally ill and/or bereaved.
- Masters degree in theology, divinity, pastoral care, philosophy, or related discipline.
- Basic Life Support certification required.
- Minimum 3 years of experience as a chaplain in a healthcare setting.
- Board certified Pastoral Counselor.
- Knowledge of diverse religious and cultural practices, healthcare ethics, and grief counseling techniques.
