Job Summary:

To provide and coordinate chaplain services for hospice patients and families, ensuring compliance with Medicare Hospice Conditions of Participation and accepted standards of practice. Integrate spiritual care into the interdisciplinary team's work, ensuring patients and families receive spiritual support, comfort, and guidance at the end of life, consistent with their spiritual traditions and values.

Key Responsibilities:
  • Provide spiritual support and counseling services through home visits and individual/group counseling, coordinating spiritual care for terminally ill patients and their families.
  • Assess spiritual needs, issues, and concerns of hospice patients and families, documenting assessments and implementing interventions.
  • Ensure staff and volunteer competency in spiritual support and needs assessment, providing training and education through departmental and community resources.
  • Develop and coordinate spiritual care resources, including literature, support groups, mentors, community resources, and educational rituals.
  • Measure and monitor the quantity and quality of pastoral care and spiritual support services, developing performance measures to improve patient and program goals.
  • Provide bereavement support to surviving family members for up to 13 months after patient death, including phone support, in-home visitations, and bereavement support groups.
Qualifications:
  • Minimum two years of experience providing spiritual care services in a healthcare setting.
  • Minimum one year of professional experience providing services to the terminally ill and/or bereaved.
  • Masters degree in theology, divinity, pastoral care, philosophy, or related discipline.
  • Drivers License (in location where applicable) required at hire.
  • Basic Life Support required at hire.
Additional Requirements:
  • Completion of at least four units of Clinical Pastoral Education Program (CPE).
  • Thorough knowledge of spiritual, emotional, and general care needs of patients and families at the end of life.
  • Proficiency in delivering emotional, spiritual, grief counseling, and support group services.
  • Demonstrates customer-focused service skills, leadership, and public speaking abilities.
Preferred Qualifications:
  • Minimum three years of service within a healthcare setting as a chaplain.
  • Minimum two years of working as a member of a hospice interdisciplinary team.
  • Board certified Pastoral Counselor.
  • Knowledge of diverse religious cultural practices, health care ethics, grief, and bereavement counseling techniques.
