Position Summary:

The role of the Home Care Coordinator is pivotal in delivering both direct and indirect administrative and clerical support to the clinical teams dedicated to serving adult patients with special needs within our community. This position acts as a vital link between clinical teams, various staff members, external agencies, care facilities, and caregivers, enhancing the efficiency and effectiveness of patient care.

Key Responsibilities

Care Coordination:

  • Organize and distribute incoming paperwork to the relevant providers, ensuring timely processing.
  • Schedule patient appointments and manage check-ins effectively.
  • Update necessary documentation as required.
  • Facilitate communication regarding patient changes at home.
  • Coordinate annual immunizations and TB testing.
  • Monitor the usage of patient portals.
  • Serve as the primary liaison between the health center and patient homes.

Patient Scheduling and Registration:

  • Collaborate with triage nurses to ensure prompt patient visits.
  • Accurately schedule appointments, optimizing clinician availability.
  • Track no-shows and cancellations to fill open slots efficiently.
  • Pre-register patients, documenting all necessary information.
  • Manage daily cancellations in the scheduling system.

Check-In Duties:

  • Welcome patients and visitors with professionalism and courtesy.
  • Accurately input demographic and financial data into the system.
  • Handle financial transactions, including co-payments, during visits.
  • Assist patients in obtaining records from other healthcare facilities.

Check-Out Responsibilities:

  • Process financial transactions at the conclusion of visits.
  • Ensure daily financial reconciliations are completed accurately.

Phone Management:

  • Answer phone calls, managing inquiries and taking messages as needed.
  • Maintain professional boundaries regarding medical advice and triage, directing calls to appropriate personnel.
  • Monitor voicemail for medication refill requests and respond accordingly.

Patient Program Coordination:

  • Assess the financial status of patients to determine eligibility for programs.
  • Assist patients with enrollment applications and follow up on pending applications.
  • Educate patients about their financial responsibilities and health center policies.
  • Participate in training sessions as required.

Health Information Services (if applicable):

  • Maintain patient records within the electronic medical record system.
  • Retrieve and deliver patient charts as requested by clinical teams.
  • Perform data entry and retrieval tasks as necessary.
  • Assist patients with completing medical records release forms.

Additional Responsibilities:

  • Consult regularly with supervisors and clinical teams regarding patient information.
  • Communicate effectively with clinical staff about scheduling and patient requests.
  • Ensure confidentiality of all medical records and patient information.
  • Attend relevant training sessions to stay updated on financial programs.
