Palliative Care Nurse

Central Coast Home Health & Hospice is seeking a compassionate and skilled Palliative Care Nurse to join our team in the Santa Barbara area. As a Palliative Care Nurse, you will provide high-quality care to patients with life-limiting diseases, focusing on symptom management, advance directives, and education regarding disease processes.
Key Responsibilities:- Provide palliative care to patients in their homes, working collaboratively with a multidisciplinary team
- Assess patient needs and develop relationships with patients and families
- Manage symptoms, advance directives, and educate patients and families about disease processes
- Work independently in a home-visit setting and effectively communicate with elderly and chronically ill patients and families
- Current California RN License
- At least one year of professional nursing experience
- CPR certified (Healthcare Provider)
- Home Health and/or Hospice experience is a plus
- Reliable vehicle to drive for work
- Current Driver's license and clean driving record
- Have and maintain auto insurance coverage

Compensation for routine visits ranges from $56.00-$65.00 per visit; evaluations range from $95.00-$110.50 per visit; and admissions range from $168.00-$195.00 per visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
