Job Summary

We are seeking a highly skilled Behavioral Health Clinician to join our team at Dartmouth Health. As a key member of our primary care team, you will provide high-quality behavioral healthcare services to patients in need.

Responsibilities
  • Collaborate with primary care teams to address behavioral health needs of patients using the Collaborative Care model.
  • Conduct triage visits and initial evaluations to clarify behavioral health concerns and develop individualized care plans.
  • Enroll patients into the collaborative care program, including in-person and telephone outreach visits for short-term episodes of care.
  • Utilize evidence-based clinical procedures to deliver care in a standardized format and measure patient symptoms and response to treatment.
  • Effectively maintain and manage patient registries to track progress and report program-focused metrics.
  • Provide limited face-to-face, short-term counseling, including problem-solving therapy, brief CBT, motivational interviewing, or brief supportive therapy.
  • Proactively communicate with patients, primary care team members, and consulting psychiatrists to aid in effective, patient-centered, team-based care.
  • Assist patients in need of higher levels of care or specialty mental health services to connect with appropriate treatment resources.
Qualifications
  • Master's degree in Social Work, Mental Health Counseling, Marriage & Family Counseling/Therapy, or Addiction Counseling from an accredited institution of higher education.
  • Must be able to demonstrate that he or she has provided psychological services on an inpatient, outpatient, or consultative basis in the past 12 months.
  • Required licensure/certifications include Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
Work Environment

This is a full-time, 40-hour/week benefitted position located at our Manchester clinic. The position will be a hybrid role, working three days a week in the office and two days a week remotely. We anticipate spending the first four to six months on-site while orienting to the position before moving to a hybrid schedule.
