POSITION OVERVIEW

The role involves delivering direct nursing care to patients under the guidance of a Registered Nurse.

KEY RESPONSIBILITIES

- Communicate significant observations, alterations, and responses concerning patients to the designated personnel.

- Alert the team leader, charge nurse, or physician regarding patient observations.

- Maintain accurate documentation in patient records regarding observations, changes, and reactions, ensuring timely reporting to the appropriate personnel.

- Relay patient conditions to the team leader during shift changes.

- Conduct regular patient rounds to ensure safety, focusing on fall prevention, restraint minimization, and safe medication administration.

- Attend to the comfort and pain management needs of patients, demonstrating empathy and understanding towards patients and their families.

- Provide direct patient care utilizing nursing processes and technical skills in accordance with hospital policies and procedures, as evidenced by both observation and documentation.

- Respond to call lights and assist patients with personal hygiene and daily living activities.

- Aid in the admission, transfer, and discharge processes of patients.

- Ensure patient comfort and safety throughout care.

- Document all observed symptoms, reactions, treatments, and changes in patient conditions, ensuring proper verbal reports and charting procedures are followed.

- Identify the educational needs of patients and/or significant others, collaborating with the Registered Nurse for effective education and rehabilitation.

- Utilize appropriate abbreviations in documentation.

- Record patient responses and significant other feedback.

- Contribute to the development of individualized nursing care plans under the supervision of the Registered Nurse, ensuring ongoing updates are documented accurately.

- Demonstrate knowledge of and adherence to hospital and nursing department policies and procedures.

- Assist physicians during patient rounds, procedures, and examinations, documenting appropriately in patient records.

- Administer medications, including IVs, following hospital protocols and documenting accordingly.

- Exhibit competency in IV insertion and tubing changes, ensuring proper use of medication dispensing systems and documentation of narcotic waste.

- Comply with patient safety standards in medication administration.

- Participate in the orientation of new staff and the clinical training of students.

- Attend mandatory in-service training and continuing education programs.

- Engage in unit meetings and stay informed through notices and memos.

- Contribute to nursing service committees as needed.

SPECIFIC JOB REQUIREMENTS

Physical Demands:
Medium Work - Involves lifting, carrying, pushing, and pulling weights occasionally, frequently, or up to 10 lbs. constantly.

Mobility:
Frequent bending, reaching, handling, and fine motor skills are required.

Communication:
Regular verbal communication is necessary to convey information and instructions clearly.

Vision:
Clear vision is required for tasks performed at close range.

EDUCATIONAL REQUIREMENTS


Completion of an accredited School of Nursing is essential, with additional requirements for specialty units as per policy.

A current State Nursing License is mandatory.

Essential Communication Skills:
- Provide clear instructions and explanations to patients and their families.

- Collaborate effectively with other healthcare team members.

- Maintain a current BLS Healthcare Provider certification, with potential ACLS certification required based on departmental needs.

WORK ENVIRONMENT

The position is set in a standard patient care environment with minimal exposure to excessive noise, dust, and temperature fluctuations. The role involves significant standing or walking, with potential exposure to unpleasant patient care activities and communicable diseases.

Overtime, weekend, and holiday shifts may be required.
