About the Role

We are seeking a highly skilled and compassionate External Case Manager to join our team at Complete Care Management. As a key member of our post-acute care team, you will play a vital role in ensuring seamless transitions for patients from hospitals to our care facility.

Key Responsibilities
  • Patient Assessment: Conduct thorough assessments of patients being discharged from hospitals to determine their care needs in the post-acute setting.
  • Transition Planning: Collaborate with hospital staff, patients, and families to develop comprehensive transition plans that meet their unique needs.
  • Communication: Facilitate effective communication between hospital and facility staff, patients, and families to ensure a smooth transition process.
  • Patient Education: Provide patients and their families with education and support to ensure they are prepared for their transition to our care facility.
  • Market Development: Identify opportunities for increased market share from assigned hospital accounts and develop strategies to expand our reach.
Requirements
  • Education: Bachelor's degree in healthcare administration, business, or a related field.
  • Experience: 3-5 years of experience as an Admissions Director, Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital.
  • Skills: Strong communication and interpersonal skills, knowledge of healthcare regulations and standards.
