Job Summary:

As a Post-Acute Care Transition Specialist with Complete Care at Willow Creek, you will play a pivotal role in ensuring seamless transitions for patients from hospitals to our post-acute care facility. Your dedication and compassion will have a profound impact on the lives of our residents, ensuring they receive the exceptional care they deserve. We are seeking a high-caliber ECM to focus on Robert Wood Johnson University Hospital in New Brunswick and St. Peter's University Hospital.

Responsibilities & Qualifications of a Post-Acute Care Transition Specialist:
  • Assess patients being discharged from hospitals to determine their care needs in the post-acute setting.
  • Collaborate with hospital staff, patients, and families to develop comprehensive transition plans.
  • Facilitate effective communication between hospital and facility staff, patients, and families.
  • Provide patient and family education.
  • Identify opportunities for increased market share from assigned hospital accounts.
  • Maintain excellent organizational skills.
  • Ensure timely communication and acceptance of hospital referrals.

Qualifications:
  • Bachelor's degree in healthcare administration, business, or a related field (preferred).
  • 3-5 years of experience as an Admissions Director, Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital.
  • Strong communication and interpersonal skills.
  • Knowledge of healthcare regulations and standards.
