Job Summary

The Field Intake Nurse is a critical role within our organization, responsible for assessing a member's eligibility for the Managed Long Term Care program. This position requires a unique blend of clinical skills, business acumen, and exceptional communication abilities.

Key Responsibilities
  • Conduct comprehensive assessments of potential members in their place of residence, utilizing a NYSDOH-approved assessment tool (UASNY) to determine eligibility and initial plan of care.
  • Effectively complete the required assessment form (UASNY) and other necessary documents within the required timeframes, adhering to our established policies and procedures, as well as State and Federal guidelines and regulations.
  • Communicate plan benefits clearly and easily to prospects and referral sources, showcasing a deep understanding of our program and services.
  • Submit thorough and high-quality documentation in a timely manner, ensuring accuracy and attention to detail.
  • Recognize quality of care issues and escalate them appropriately, communicating effectively with coordinators and supervisors as needed.
  • Maintain member confidentiality and comply with HIPAA requirements, upholding the highest standards of professionalism.
Requirements
  • Graduate of a 2-4 year accredited nursing program.
  • Active and unrestricted NYS RN license.
  • Strong written and verbal communication skills.
  • Proficient in the use of Microsoft Office Suite tools.
  • Ability to travel within assigned areas.
