Job Overview:
The Clinical Care Coordinator (CCC) I is an integral part of the healthcare team, reporting directly to the Case Management Manager. Under the supervision of the Clinical Care Team Leader, the CCC I collaborates with clinical professionals to establish a comprehensive care plan aimed at achieving optimal health outcomes and facilitating a smooth discharge process.

Key Responsibilities:
- Manage and oversee the care plan throughout the patient’s journey within the healthcare system, ensuring efficient transitions and continuity of care.
- Act as a clinical authority in discharge planning, resource allocation, and case management, providing expert guidance to both patients and staff.

Work Environment:
- This position typically requires a commitment of 40 hours per week, primarily during daytime hours, with occasional weekend and holiday shifts.
- A competitive sign-on bonus is available for qualified external candidates.

Qualifications:
- A Bachelor’s degree in Nursing from an accredited institution is required, along with a minimum of three years of experience as a Registered Nurse.
- Prior leadership experience and familiarity with case management or utilization review are preferred, as well as certification in Case Management.

Licensure and Certifications:
- Must hold a valid Registered Nurse license in South Carolina or a compact state.
- Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification is mandatory.

Physical Requirements:
- The role necessitates the ability to perform various physical tasks, including standing, walking, and lifting, while maintaining a high level of patient care.
- Candidates should be able to handle physical demands such as lifting patients and performing fine motor skills.
