About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Hospice team as an Admissions Nurse. As a key member of our interdisciplinary team, you will play a vital role in coordinating the admissions process for our patients and their families.

Responsibilities
  • Coordinate admissions of hospice patients by conducting skilled assessments of their physical, psychosocial, and spiritual needs.
  • Develop and implement individualized patient care plans in collaboration with the attending physician, medical director, and interdisciplinary team.
  • Provide skilled interventions and education to patients and caregivers to promote optimum symptom control and patient comfort.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress and outcomes accurately and timely, in accordance with our care center policies.
Requirements
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience.
  • Current CPR certification.
