Key Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services
  • Identify and address the physical, psychosocial, and environmental needs of patients and their families
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
  • Provide education and support to patients and their families on self-care techniques and end-of-life care
  • Collaborate with the healthcare team to ensure seamless care coordination
Requirements
  • Current RN licensure in the state of South Carolina
  • One year of clinical experience in hospice or palliative care
  • Current CPR certification
  • Valid driver's license and reliable transportation
