Wheeler Clinic is seeking a skilled Intake Provider to join our team. As a key member of our medical team, you will provide same-day access for new patients establishing medical care or seeking medication-assisted treatment.
Job SummaryThe Intake Provider will function as a member of a medical team, providing same-day access for new patients coming to Wheeler to establish medical care or seeking medication-assisted treatment. This role requires strong clinical skills, excellent communication, and a patient-centered approach.
Responsibilities- Conduct medical evaluations and formulate accurate diagnoses in accordance with the International Classification of Diseases.
- Manage prescribed medication, perform physical health examinations, conduct preventive screening and care, and develop and present clear clinical impressions and recommendations.
- Develop and document clear clinical formulations and medical treatment plans.
- Prescribe therapeutic interventions appropriate to identified problems and administer drugs and biologicals as required by the Clinic.
- Triages clinical emergencies and implements safety and consultation protocols.
- Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education.
- Provides requisitions for clinical studies, diagnostic tests, and laboratory studies and evaluates and interprets results to identify and assess patient's clinical problems and health care needs.
- Makes clinically appropriate referrals for consultation with specialists, subspecialists, and allied health professionals as indicated.
- Ensures all patients' medical records are completed timely and in accordance with organization policy and that all charting is comprehensive and legible.
- Works with care coordination team in designing, monitoring, and implementing care plans and communicates referrals and collaborates on development of services planning and implementation.
- Maintains consistent communication with community providers, local and state agencies, and other allied professionals and services mutually involved in client care.
- Represents the Clinic in external and internal committee meetings as assigned.
- Communicates in a professional, positive, effective, and courteous manner with client, family, and all internal and external providers.
- Meets direct service productivity goals as defined by program and demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.
- Attends and is prepared for scheduled meetings with administrative supervisor and attends and is prepared for scheduled clinical meetings and consultation with supervising physician.
- Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.
- Master's degree in nursing is required.
- Must have one (1) year of post-licensure primary care experience or urgent care or triage experience or three (3) years of other post-licensure experience.
- Exceptions can be made for students who complete an APRN rotation at Wheeler, as well as three months of Wheeler's accelerated residency program.
- Licensed Registered Nurse and Licensed Advanced Practice Registered Nurse or Physician Assistant in the State of Connecticut.
- Board Certified as an Advanced Practice Registered Nurse or Physician Assistant as appropriate for the population served.
- Active DEA controlled substance registration and an active Connecticut controlled substance registration.
- Ability to acquire and maintain certification in Basic Life Support for Healthcare Professionals no later than 90 days from date of hire.
