Licensed Clinical Social Worker

South Central Family Health Center is seeking a highly skilled Licensed Clinical Social Worker to join our team. As a key member of our healthcare team, you will provide social services and assistance to improve the social and psychological functioning of children and their families.
Responsibilities:- Counsel individuals, groups, families, or communities regarding mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care.
- Interview clients to assess their situations, capabilities, and problems to determine what services are required to meet their needs.
- Serve as a liaison between students, homes, schools, family services, child guidance clinics, courts, protective services, doctors, and other contacts to help children who face problems.
- Maintain case history records and prepare reports.
- Counsel parents with child rearing problems and develop specific behavioral change plans with patients.
- Provide skill training, patient education, and evaluate patient care plans with primary care team.
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
- Assess the clinical status of patients referred by the primary care provider.
- Assist in the detection of at-risk patients and prevent further psychological or physical deterioration.
- Consult medical director and staff clinicians as necessary and refer cases to specialty providers as appropriate.
- Maintain productivity based on established standards as discussed with supervisor.
- Accurately record patient history, exam notes, medication history, on-going care, and referrals in medical records.
- Supervise and train MSWs & ACSWs.
- Attend and participate in provider, staff, and Quality Assurance activities as required.
- MA Degree in Social Work or Psychology from an accredited University or College.
- Current licensure by the State of California Behavioral Science Examiners Board as a Licensed Clinical Social Worker.
- Current Basic Life Support (BLS).
- Minimum 3 years of independent counseling experience.
- Ability to work independently within scope of assigned tasks with minimum supervision.
- Knowledge of MediCal, MediCare, and private insurance rules & regulations pertaining to mental health services.
- Familiarity with Protective Agency reporting requirements.
- Familiarity with resources that receive and care for patients.
- Bi-lingual/bi-literate English/Spanish preferred.
- Background/training in crisis work or solution-focused therapy, brief focus interventions, or other time-limited therapy preferred.
