Job Summary: Kaiser Permanente is seeking a skilled Medical Social Worker III to provide high-quality social services to patients and their families in a medical setting.
Key Responsibilities:
- Assessment and Intervention: Conduct thorough bio-psychosocial assessments to identify patients' social, emotional, and economic needs, and develop and implement effective treatment plans to address these needs.
- Crisis Intervention: Provide crisis intervention and counseling services to patients and their families in emergency situations, including assessment, counseling, information/referral, and consultation with healthcare teams.
- Psycho-Social Intervention: Interpret patients' conditions and recommend appropriate psycho-social interventions and treatment plans.
- Cultural Sensitivity: Develop culturally sensitive assessments that reflect department standards and include reason for referral, source of referral, informant name, physical/medical issues, interpersonal and social relations, emotional/psychiatric/adjustment issues, and community planning/resources/social transition.
- Patient Education: Provide patient education on subjects related to psycho-social adjustments to medical illness, individually, in classes, or groups.
- Interdisciplinary Collaboration: Function as an effective team member, consultant, and liaison to medical, nursing, ancillary staff, and community agencies, demonstrating an interdisciplinary approach, competent follow-up, and communication of accurate information.
- Professional Conduct: Demonstrate professional behavior and interventions consistent with departmental policies and procedures, JCAHO, Title 22, and NCQA legal mandates, professional standards, NASW, ABECSW, and other Medical Center Guidelines.
Requirements:
- Education: Master's Degree in Social Work issued by a school accredited by the Counsel of Social Work Education.
- License: Licensed Clinical Social Worker (California).
- Experience: Minimum one (1) year experience working as a MSW in medical acute care setting or as part of a medical team.
- Additional Requirements: Current CA Drivers License, clear driving record for the past two (2) years, and proof of automobile insurance as required by law.
