Children's Care Manager RN

Job SummaryThe Children's Care Manager Registered Nurse will be responsible for coordinating care for medically fragile children and their families. This includes reviewing and approving plans of care, conducting assessments, and authorizing services. The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively with healthcare providers, families, and community agencies.
Key Responsibilities- Coordinate care with Health Home Coordinators and LDSS/VFCA Care Managers to ensure comprehensive treatment needs are met.
- Provide education and coaching to members' families to support their child's health and well-being.
- Conduct quarterly assessment calls with Health Home Coordinators to monitor progress and address concerns.
- Collaborate with physicians and other healthcare providers to ensure seamless care transitions and discharge planning.
- Perform telephonic care reviews and authorization determinations for pre-admission and continuing care.
- Determine the appropriate level of care for each child, taking into account their medical needs and family circumstances.
- Consult with Clinical Peer Reviewers to discuss clinical and authorization questions, alternative treatment options, and concerns regarding specific cases.
- Provide information to members' families, Health Home coordinators, LDSS/VFCA, and clinical providers regarding medical benefits and services.
- Recognize quality of care issues and report them appropriately through internal and external processes.
- Bachelor's Degree in Nursing required.
- A minimum of two years' clinical experience in an acute care setting.
- Experience using Interqual Guidelines required.
- Knowledge of medical care and HCBS services needed by medically fragile children.
- A valid NYS RN license.
