Responsibilities:

As a Care Management Associate, you will play a vital role in implementing transition plans, communicating with treatment teams, patients, and families, and collaborating with external colleagues like primary care physicians, post-acute providers, and insurance companies.

You will be responsible for:

  • Mastering discharge dispositions and delivering crucial information from Medicare, Observation, and Denials letters
  • Facilitating transportation arrangements and providing vital information to patients and representatives
  • Relaying important issues to care managers or social workers and sharing vital information with insurance companies as directed
  • Conquering the E-discharge realm by entering and executing sage transmissions of patient medical records
Requirements:

To succeed in this role, you will need:

  • Education: High school education/GED preferred with 3 years of previous medical experience or 1 year with an advanced degree
  • License/Certifications: N/A
  • Experience: Experience with community resources or insurance companies desirable
