Adventist Health is seeking a skilled RN, Care Manager to join our dynamic team. As a key member of our Care Management department, you will play a vital role in collaborating with internal and external team partners to support patients and their families across the care continuum.

Job SummaryThe RN, Care Manager will provide excellent patient care by assisting in the development, implementation, and revision of the case management program. You will act as a liaison between patients, families, nurses, physicians, and the multidisciplinary team to optimize outcomes. Your expertise will be sought by the healthcare team on specific patient items, and you will coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge.
Responsibilities- Collaborate with the care team to lead the coordination of patient care, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures.
- Focus on complex patients, frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients.
- Collaborate with acute care case managers to ensure appropriateness of ongoing care.
- Provide oversight and collect data required for regulatory and accreditation compliance.
- Manage frequent emergency department visitors by conducting a focus study review of previous and current admissions.
Bachelor's Degree in Nursing (BSN) and experience in a care management role are required. Adventist Health is committed to the safety and wellbeing of our associates and patients, and we require that all associates receive all required vaccinations.
