  • Evaluate patient needs, reporting new findings to RN and MD to inform comprehensive care plans.
  • Implement evidence-based nursing interventions and procedures within licensure guidelines, ensuring optimal patient care.
  • Maintain accurate records of patient progress, problems, and responses through ongoing evaluation and reassessment.
  • Identify necessary ancillary services and communicate effectively with RNs to coordinate patient care.
  • Collaborate with healthcare teams to facilitate access to community resources, promoting holistic patient support.
  • Document skilled care provided, ensuring transparent and accurate record-keeping.


Requirements:

  • A graduate of an approved LPN school of nursing program.
  • Current LPN license in Massachusetts.
  • CPR certification required for all LPNs working at Cape Cod Healthcare Inc.
  • At least one year of recent experience in an acute care facility setting.
  • Preferred qualifications include homecare experience and proficiency in electronic medical records systems.


Estimated Salary Range:$60,000 - $80,000 per annum, depending on qualifications and experience.
