Clinical Triage Nurse Job Description

We are seeking a skilled Clinical Triage Nurse to join our team at Cape Cod Healthcare Inc. in N. Falmouth. This is an exciting opportunity to make a difference in the lives of patients and families.

Job Summary

The Clinical Triage Nurse will be responsible for providing exceptional patient care, responding to incoming calls and messages, and exercising clinical judgment to ensure health needs and concerns are met.

Key Responsibilities:
  • Respond to incoming calls and messages in a timely manner
  • Triage patients based on assessment of their condition and availability of resources
  • Collaborate with the interdisciplinary care team to provide comprehensive care
  • Provide education to patients about utilization of community services
  • Assist clinicians with clinical procedures and take vital signs as needed
  • Maintain accurate and complete documentation in the electronic health record
  • Participate in practice and department meetings to promote continuous improvement and quality patient care
Requirements:
  • Graduate of an accredited Practical Nursing program (LPN) or school of nursing (RN)
  • Currently licensed as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Massachusetts
  • Basic Life Support Certification for Health Care Providers
  • Minimum 2 years' experience in a clinical setting preferred
  • Strong interpersonal, customer service, time management skills
Salary:

$60,000 - $80,000 per year, depending on experience.
