Job Summary

About the Role
The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing ongoing support and education to patients and their caregivers. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities
- Assess patient needs and develop individualized care plans
- Provide direct patient care, including administering medications and treatments
- Educate patients and their caregivers on disease management and self-care techniques
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Document patient information and progress in a timely and accurate manner

Requirements
- Graduate of an approved LPN school of nursing
- Current LPN license in MA
- CPR certification required
- Minimum one (1) year recent experience in an acute care facility or home healthcare setting
