Empath Health is actively seeking a Care Navigator to enhance our dedicated team.
Work Schedule: Tuesday-Saturday 10:30am-7:00pm
The Care Navigator will be responsible for:
- Coordinating service inquiries and ensuring prompt access within the Empath Health Network of Care.
- Delivering exceptional customer service with a focus on compassion, empathy, and responsiveness to the needs of all individuals who reach out to us.
- Facilitating optimal access to the Empath Health Care Continuum through various channels including telephone, website, fax, and in-person inquiries, while providing tailored information and education regarding our programs and services.
- Effectively triaging appointments using Care Navigation protocols to connect patients with the most suitable programs.
- Collaborating with patients, families, community providers, and team members to gather and input necessary information for accurate referrals into the patient's electronic medical record (EMR) in a timely and precise manner.
- Possessing extensive knowledge of community resources beyond hospice care and sharing this information with patients, families, and providers as needed.
- Providing contact details for hospices outside of the local area when appropriate.
- Ensuring the receipt and input of physician certification information into the EMR, adhering to regulatory standards.
- Monitoring, triaging, and documenting faxes received, ensuring they are forwarded to Health Information for EMR integration as necessary.
- Conducting daily audits to validate inquiries, scheduled appointments, and patients served.
- Education and/or Experience: High school diploma or GED; or one to three months of related experience and/or training; or an equivalent combination of education and experience.
- Outstanding verbal and written communication skills with patients, families, providers, and colleagues.
- Strong multi-tasking and critical thinking abilities.
- Proficient typing and computer input skills.
- Familiarity with Microsoft Office products (MS Outlook, Word, Excel); knowledge of Suncoast Solutions is preferred.
- Understanding of HIPAA compliance regulations.
- Basic knowledge of healthcare and community resources outside of hospice.
- Fundamental understanding of medical terminology.
- Basic awareness of healthcare reimbursement processes.
- Willingness to assist with Care Navigation coverage as needed outside of regular shifts.
