Primary Care Physician Leader

ChenMed is a physician-led organization dedicated to providing exceptional primary care to seniors. We're seeking a highly skilled and compassionate Primary Care Physician Leader to join our team.
As a Lead Physician for Clinical Excellence, you will be responsible for delivering high-quality patient care, leading a team of healthcare professionals, and driving clinical excellence in our centers. You will work closely with our center and market leadership to ensure compliance with guidelines and participate in risk and quality management programs.
Key Responsibilities:
- Provide exceptional patient care, including assessment, diagnosis, treatment, and management of acute and chronic health needs.
- Lead a team of healthcare professionals, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Develop and implement strategies to improve patient outcomes, quality measures, and clinical excellence.
- Collaborate with hospitalists, specialists, and other healthcare professionals to ensure comprehensive patient care.
- Participate in risk and quality management programs, clinical meetings, and other meetings as required.
Requirements:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current State Medical License or ability to obtain one.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross.
