Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team. As a Primary Care Physician at ChenMed, you will be responsible for providing high-quality, patient-centered care to our senior patients. You will work closely with our care teams to develop and implement personalized care plans that address the unique needs of each patient.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, including geriatric patients.
- Develop and implement evidence-based treatment plans that address the physical, emotional, and social needs of patients.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Lead and participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with the latest medical research and best practices to provide the highest level of care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
