Lead Primary Care Physician

Role Summary: The Lead Primary Care Physician (Lead PCP) serves as the foundational step in our clinical leadership pathway. This role entails accountability for patient outcomes, delivering high-quality clinical care, and ensuring cost-effectiveness for a panel of at least 360 patients.
Patient Engagement: The Lead PCP plays a crucial role in influencing patient outcomes by fostering trusting relationships and facilitating behavior change.
- Acts as a health coach, guiding patients in setting and achieving short- and long-term health objectives.
- Leads weekly Super Huddles and Transforming Care Meetings, while supporting center leadership in promoting a positive workplace culture.
Performance Metrics: Our organization is focused on outcomes and value-based care. Key performance indicators for the Lead PCP include patient admissions per thousand, appointment availability, and patient experience metrics.
Cultural Leadership: As leaders within our centers, Lead PCPs are expected to champion a culture of compassion, accountability, and enthusiasm, collaborating closely with center leadership.
Collaboration and Compliance: The Lead PCP will work in tandem with center and market leadership to ensure adherence to clinical guidelines and participate in quality management initiatives.
Clinical Responsibilities: This role involves providing comprehensive care for patients with both acute and chronic conditions, engaging with hospitalists when necessary, and leading a care team that includes medical assistants and care coordinators.
Accessibility: The Lead PCP is expected to be readily available to patients, accommodating walk-in visits and responding to inquiries promptly.
Commitment to Quality: We believe our patients deserve the highest quality of care, which requires a dedication to continuous improvement in clinical practices and operational efficiency.
Qualifications: Candidates must possess an MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty, along with a valid State Medical License. Board certification in the relevant specialty is preferred.
