Job Summary

We are seeking a highly motivated and detail-oriented Community Outreach Coordinator to join our team at Community Health Group. As a key member of our outreach team, you will play a critical role in promoting our Medi-Cal health plan and providing exceptional customer service to our members.

Key Responsibilities
  • Provider Relations: Establish and maintain positive relationships with healthcare providers, ensuring they are informed about our plan's policies, procedures, and contractual changes.
  • Community Outreach: Develop and implement outreach strategies to engage with community-based organizations, Medi-Cal beneficiaries, and the general public, promoting our health plan and services.
  • Education and Training: Provide education and training to providers on Medi-Cal managed care and Community Health Group programs and services.
  • Issue Resolution: Address and resolve provider issues related to day-to-day operations, ensuring timely and efficient resolution.
  • Collaboration: Work closely with various departments, including Claims, Customer Service, Contracting, and others, to ensure seamless communication and issue resolution.
  • Event Planning: Organize and participate in community health fairs and other events to promote our health plan and services.
  • Enrollment: Enroll Medi-Cal beneficiaries interested in joining our health plan.
  • Professional Development: Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • Education: Bachelor's degree preferred.
  • Experience: One year successful sales experience and two years successful community outreach experience, preferably related to Medi-Cal and/or Medicare.
  • Skills: Bilingual ability in Spanish or Vietnamese required, excellent written, verbal, and telephone skills, ability to analyze program requirements and regulations, and identify issues and resolve complex problems.
  • Physical Requirements: Frequent driving, public speaking, and ability to lift and carry up to 30 pounds.
