Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our Caminos program. As a key member of our team, you will work closely with vulnerable children and their families to provide support and guidance throughout the transition process.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. Fluency in Spanish is a must, as you will be working with children and families who are primarily Spanish-speaking.

Key Responsibilities
  • Act as a primary point of contact and liaison for children, families, and external agencies
  • Complete admission requirements, assessments, and Individual Service Plans within regulatory timeframes
  • Coordinate the timely collection and completion of required documentation for children in care
  • Ensure children receive the full range of services identified in the ISP in an integrated and comprehensive manner
  • Provide assessment of the relationship between children and sponsors, utilizing safe screening methods to ensure sponsors are appropriate
  • Provide direct assistance to sponsors to ensure thorough completion of documentation required for timely release of children
  • Create required time-sensitive reports and documentation of progress toward discharge
  • Provide daily, weekly, and monthly submission of required updates to internal and external team members
  • Facilitate regular case management meetings for individuals, groups, and families
  • Coordinate with Everstand staff members and community resources to address children's service needs
  • Evaluate children's progress toward goal attainment on a designated basis in line with individual service planning and discharge plan goals
  • Take the lead role in making transfer and release recommendations, as well as coordinating the release of children from care in accordance with regulatory timeframes
  • Maintain effective working relationships with families, children, colleagues, consultants, and community agencies
  • Ensure assigned case records meet documentation standards as defined by both federal and state regulations
  • Ensure required contacts with Treatment Foster Care parents, both by phone and in person, take place and are documented in accordance with regulatory expectations
  • Provide in-ratio supervision of children as required
  • Identify supervision and/or professional training needs to supervisor as they arise
  • Ensure filing compliance in accordance with ORR and state regulations related to positions documentation responsibility

Available shifts: Front-End Sunday-Thursday (9am - 5pm). This position regularly requires flexible work hours based on the needs of the program to include evening and weekend work.

Required Education and Experience:

  • Bachelor's degree in behavioral sciences or human services (no prior experience required)
  • Candidates must be fluent in Spanish and English (written and verbal)

Preferred Education and Experience:

  • Child Welfare and Case Management experience preferred

Required Language Skills:

  • Spanish-speaking and reading are a must. We accept candidates who have varying degrees of fluency.
  • Everstand is OK with applicants for whom English is a second language, however, please note that documentation of notes and all competency-based exams (e.g. First Aid and CPR class) are in English.
