Job Summary

CVS Health is seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide exceptional patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Key Responsibilities
  • Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients
  • Collaborate with the clinical care team to ensure seamless care coordination and communication
  • Develop and implement patient-centered care plans, incorporating evidence-based practices and guidelines
  • Manage patient relationships, providing education and support to promote health and wellness
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices
Requirements
  • Master's degree in Family Nurse Practitioner program or Master of Science in Physician Assistant Studies
  • Current National Board Certification and State of Employment license to practice in the Advanced Practice role
  • Minimum 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions
  • Effective verbal, written, and electronic communication skills
  • Outstanding organizational skills and ability to multi-task
