Job Summary

CVS Health is seeking a highly skilled Nurse Practitioner to join our Primary Care team in San Antonio. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Responsibilities
  • Provide accurate and complete assessments, diagnoses, treatments, and management of health problems
  • Counsel and educate patients and families on health maintenance and respond to patient care inquiries
  • Evaluate and treat patients' medical needs with safe and high-quality treatment
  • Document all patient care within an electronic health record in accordance with current policies and procedures
  • Collaborate with clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care
  • Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies
Requirements
  • Master's degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role
  • A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
  • Current National Board Certification in Family Practice
Working Environment

The Primary Care NP is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligently, and hear patient responses. Specific vision abilities include the ability to view and read a computer screen and other electronic devices.
