Job Summary

CVS Health is seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide evidenced-based care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Key Responsibilities
  • Provide accurate and comprehensive patient assessments, diagnoses, and treatment plans
  • Develop and implement individualized care plans, taking into account patient needs and preferences
  • Collaborate with the clinical care team to ensure seamless care coordination and communication
  • Participate in quality improvement initiatives to enhance patient outcomes and experience
  • Stay up-to-date with the latest clinical guidelines and best practices in primary care
Requirements
  • Master's degree in Family Nurse Practitioner or Physician Assistant Studies
  • Current National Board Certification and State of Employment license to practice in the Advanced Practice role
  • Minimum 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
