Nurse Practitioner/PA

CVS Health Careers is seeking a highly skilled and compassionate Nurse Practitioner/PA to join our Primary Care team. As a key member of our clinical care team, you will provide evidenced-based care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Key Responsibilities- Provide accurate assessment, diagnosis, treatment, and management of health problems, health counseling, and disposition planning.
- Use knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care.
- Counsel and educate patients and families on health maintenance and respond to patient care inquiries.
- Evaluate and treat patients' medical needs with safe and high-quality treatment.
- Document all patient care within an electronic health record in accordance with current policies and procedures.
- Work as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients.
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice preferred.
- Successful completion of CVS Minute Clinic primary care training program.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Ability to work remotely in a clinical care team culture.
- Ability to work without direct supervision and practice autonomously.
- Is proficient with information management and technology.
- Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
