Full-time/Day Shift LPN - 8 Hour Shifts During Weekdays

Additional duties include managing FMLA/STD documentation and conducting admissions assessments for our facility.

Key Responsibilities:

OBJECTIVE:


Deliver nursing care and services in accordance with the standards and scope of nursing practice, as outlined by the state Nurse Practice Act regulations, policies, procedures, and protocols, along with facility and corporate guidelines.

Exhibit a positive, empathetic, and professional demeanor towards clients at all times. Acknowledge that patient safety is of utmost importance.

CORE FUNCTIONS:

  • Assess patient health and provide basic care utilizing the nursing process within the LPN/LVN scope of practice to achieve care plan objectives and goals.
  • Exemplify expectations related to customer service and demonstrate urgency regarding the significance of patient safety.
  • Collaborate with team members to ensure that patients' physical, biopsychosocial, age, developmental, and cultural needs are addressed, and when unmet, recognize and work to resolve client concerns.
  • Ensure the basic comfort of patients, implementing nursing interventions tailored to their individual needs as outlined in the care plan.
  • Apply appropriate interventions to facilitate patient progress towards wellness and maintain overall health.
  • Discuss care provided with patients and actively listen to their concerns. Report any changes in status and concerns to registered nurses and physicians.
  • Communicate therapeutically and professionally with patients, visitors, colleagues, and others involved in the patient's care in an effective manner.
  • Ensure the safe administration of medications in compliance with policies, procedures, physician orders, and current practices.
  • Uphold standards of professional nursing practice in accordance with facility policies, external governing and credentialing bodies, performance improvement standards, and psychiatric nursing standards.
  • Responsible for thorough and accurate documentation of patient symptoms, responses, and progress.

ADDITIONAL FUNCTIONS:

  • Perform other duties and tasks as assigned.

Qualifications:

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Graduation from an accredited nursing program is required.
  • One or more years of nursing experience in the clinical area of service provided by the facility is preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

  • Current LPN/LVN license as mandated by state regulations.
  • CPR and de-escalation/restraint certification required (training available upon hire and provided by the facility).
  • First aid certification may be required based on state or facility guidelines.
