Full-time/Day Shift LPN - 8 Hour Shifts

Key Responsibilities:

OBJECTIVE:


Deliver nursing care and services in alignment with the standards and scope of nursing practice, as outlined by state regulations, facility policies, and corporate guidelines.

Maintain a positive, empathetic, and professional demeanor towards all clients. Prioritize patient safety at all times.

PRIMARY DUTIES:

  • Assess patient health and provide essential care utilizing the nursing process within the LPN/LVN scope of practice to achieve care plan objectives.
  • Exemplify customer service expectations and demonstrate urgency regarding patient safety.
  • Collaborate with team members to ensure comprehensive care addressing patients' physical, psychological, developmental, and cultural needs, and proactively resolve any concerns.
  • Ensure patient comfort by implementing individualized nursing interventions as outlined in the care plan.
  • Apply appropriate strategies to support patient wellness and maintenance of health.
  • Engage with patients regarding their care, actively listening to their concerns and reporting any changes to registered nurses and physicians.
  • Communicate effectively and professionally with patients, visitors, colleagues, and other stakeholders involved in patient care.
  • Administer medications safely in accordance with established policies, physician orders, and current practices.
  • Uphold professional nursing standards in accordance with facility policies, external governing bodies, and performance improvement standards.
  • Ensure thorough and accurate documentation of patient symptoms, responses, and progress.

ADDITIONAL DUTIES:

  • Carry out other functions and tasks as assigned.

Qualifications:

EDUCATION/EXPERIENCE/SKILLS:

  • Graduation from an accredited nursing program is required.
  • One or more years of nursing experience in a clinical setting is preferred.

CERTIFICATIONS/LICENSES:

  • Current LPN/LVN license as mandated by state regulations.
  • CPR and de-escalation/restraint certification required (training provided upon hire).
  • First aid certification may be required based on state or facility guidelines.
