Responsibilities:
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
  • Administer medication and treatment as prescribed by the physician, and provide education to patients and families on their care plans.
  • Provide emotional support and counseling to patients and families, and facilitate their involvement in the care planning process.
  • Maintain accurate and timely clinical notes, and participate in interdisciplinary team meetings to discuss patient progress and address any concerns.
Qualifications:
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in hospice or a similar setting is highly desirable.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
