Our Support Coordinators come from diverse backgrounds, yet all receive comprehensive training in the field.

By channeling your dedication to service, you will contribute to our high standards of care for our members.


This engaging role is conducted in a hybrid setting, which includes essential visits with the individuals and families you support, participation in regular training sessions and meetings at the office, and remote work.

If your support extends beyond urban areas, a valid driver's license, dependable transportation, and suitable insurance will be necessary.

Our membership is varied, and so is our workforce.

Key Responsibilities:
Advocate for our members, ensuring informed decision-making, consent, and appropriate guardianship.


Schedule, facilitate, and collaborate with our members and their interdisciplinary teams to conduct meetings and assessments, ensuring the development of a comprehensive, person-centered Life Plan that aligns with the individual's needs and aspirations.

Implement, update, and monitor Life Plans while facilitating individualized reviews and approval processes.


Ensure the integration of all necessary and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based resources.

Maintain ongoing communication with key individuals in a member's life.

Ensure timely submission of all required documentation, including Life Plans and progress notes.


Assist members in maintaining benefits such as Social Security, Supplemental Security Income, Medicaid, Medicare, and Food Stamps.

Support members in resolving everyday challenges related to housing, utilities, the legal system, and overall safety.

Report any observed or reported abuse or neglect immediately.

Qualifications:
Support Coordinators serving individuals with I/DD must meet the following criteria:

A Bachelor's degree with 2+ years of relevant experience, OR

A Registered Nurse license with 2+ years of relevant experience, which may include any employment experience and is not limited to case management/service coordination duties, OR

A Master's degree with 1+ year of relevant experience.

Strong communication skills, both verbal and written, along with excellent interpersonal and organizational abilities are essential.
