Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Lifespace Communities. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our residents.

Key Responsibilities
  • Participate in resident care planning and coordination, ensuring seamless delivery of services.
  • Document resident conditions and nursing needs accurately and in a timely manner, reporting pertinent observations and reactions.
  • Develop and direct nursing assistant assignments, adjusting based on census and level of care required.
  • Administer medications, document reactions, and initiate physician orders as necessary.
  • Facilitate communication with families regarding medication changes and resident updates.
  • Establish protocols to ensure care plans are reviewed and updated regularly.
  • Coordinate admissions, discharges, and transfers to deliver exceptional customer service.
Requirements
  • Licensed Practical Nurse graduate from an accredited nursing program.
  • Certifications and licensure required by state regulations.
