Job Summary:

This role involves evaluating, planning, implementing, and documenting nursing care for patients in alignment with organizational policies and professional nursing standards. The nurse will utilize a structured framework for professional nursing practice and development.

This position holds accountability for the quality of nursing services provided by oneself or by those under their supervision.

The nurse will apply specialized knowledge, judgment, and nursing skills essential for assessing data and delivering care tailored to the physical and developmental age of assigned patients.


Minimum Qualifications:
Education:
Graduate of an accredited nursing program.
Credentials:

Must possess a current, valid RN license in the state of practice, a temporary RN license, or compact RN licensure for the current state of practice. BLS certification is mandatory. ACLS certification is required; PALS, NIHSS, and completion of CPI are preferred. Additional certifications or continuing education may be necessary based on the specific area of practice.


Physical Demands:

Must be able to stand, walk, bend, squat, reach, and stretch frequently. Requires physical agility and adequate reaction time to respond swiftly and appropriately to unexpected patient care needs. Adequate hearing and visual acuity, including color vision, are necessary. Fine motor skills, eye-hand coordination, and the ability to grasp and handle objects are essential. The role may require lifting up to 50 pounds and using proper body mechanics to assist patients in ambulating or transferring.

Standard precautions must be employed due to potential exposure to blood and bodily fluids. Effective communication through reading, writing, and speaking is essential, and periodic use of a personal computer may be required.


Essential Functions:

Conducts comprehensive assessments of patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical examination with the patient/family, ensuring timely completion. Analyzes available information from other healthcare team members and reviews diagnostics and laboratory data, reporting abnormal results to physicians and other caregivers.

Develops a care plan, including discharge planning, utilizing assessment data and input from patients, families, and the healthcare team. Initiates care plans based on patient-specific needs and assessment data, ensuring goals are realistic, measurable, and collaboratively developed with the patient/family.

Plans care in collaboration with multidisciplinary team members and revises care plans to reflect changing patient needs based on evaluations. Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.

Provides care according to physician orders and nursing plans, adhering to policies, standards of care, and regulatory requirements. Appropriately delegates tasks and supervises nursing care provided by other licensed nurses and personnel.

Ensures continuity of care by effectively communicating patient status to other caregivers. Evaluates patient and family responses to care and education, revising care plans as necessary. Assesses patient progress towards goals and expected outcomes in collaboration with other healthcare team members.

Documents all aspects of assessment, planning, implementation, and evaluation in the patient record, ensuring documentation is legible, timely, and compliant with policy. Documentation should reflect objective/subjective data, nursing interventions, and patient responses to treatment.

Engages in evidence-based care and may participate in research activities within clinical practice. Contributes to shared leadership within the unit or facility and participates in the education and orientation of facility personnel, patients, students, families, and visitors. Supports the growth of others through mentoring and precepting when appropriate.

Actively participates in initiatives aimed at improving national nursing quality indicators and outcomes, which may include involvement in professional organizations.
