This role involves evaluating, planning, executing, and documenting nursing care for patients in alignment with organizational guidelines and the standards of professional nursing practice. The position is responsible for the quality of nursing services provided by oneself or those under their supervision.

This role requires specialized knowledge, judgment, and nursing skills to assess data and develop, deliver, and evaluate care that is suitable for the physical and developmental stages of assigned patients.

Minimum Qualifications:
Education:
Graduate of an accredited nursing program.
Credentials:

Must hold a current, valid RN license in the state of practice, a temporary RN license in the state of practice, or compact RN licensure for the current state of practice. BLS certification is mandatory. ACLS is required; PALS, NIHSS, and completion of CPI are preferred. Additional certifications or continuing education may be necessary based on the area of practice.

Physical Demands:
Must be able to stand, walk, bend, squat, reach, and stretch frequently.
Requires physical agility and the ability to respond swiftly and appropriately to unexpected patient care situations.
Must have adequate hearing and visual acuity, including color vision.
Requires fine motor skills, good eye-hand coordination, and the ability to grasp and manipulate objects.

Must be able to use proper body mechanics to assist patients in moving, transferring in and out of beds, chairs, or wheelchairs. May need to lift up to 50 pounds.
Must adhere to standard precautions due to potential exposure to blood and bodily fluids.
Must be able to communicate effectively through reading, writing, and speaking in person or on the phone.
May require occasional use of a personal computer.

Essential Functions:
Conducts assessments of patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical condition with the patient/family and ensures completion within the appropriate timeframe. Analyzes available information from other healthcare team members. Reviews diagnostic and laboratory data, reporting abnormal results to the physician(s) and other relevant caregivers. Completes assessments and reassessments according to patient needs and as outlined in policy.

Develops a comprehensive care plan, including discharge planning, utilizing assessment data and input from the patient, family, and healthcare team.

Initiates care plans based on patient-specific needs, assessment data, and the medical plan of care. Goals for patients are realistic, measurable, and developed collaboratively with the patient/family. Takes into account the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient in the care plan. Collaborates with multidisciplinary team members in planning care. Reviews and updates the care plan to reflect changing patient needs based on evaluations of the patient's status.

Implements the care plan through direct patient care, coordination, delegation, and supervision of the healthcare team's activities.

Provides care in accordance with physician orders and the nursing care plan, adhering to policies, procedures, standards of care, and regulatory agency requirements.

Delegates appropriately and supervises the provision of care to patients by other licensed nurses and personnel.

Ensures continuity of care by accurately and thoroughly communicating patient status to other caregivers.

Evaluates the patient's and family's responses to care and education, adjusting the care plan as necessary. Assesses patient progress towards goals and expected outcomes in collaboration with other healthcare team members. Evaluates the effectiveness of patient education.
Documents assessments, planning, implementation, and evaluations in the patient record. Documentation must be clear, timely, and in accordance with policy, reflecting objective/subjective data, nursing interventions, and patient responses to treatment. Accurately notes physician orders in a timely manner.
Provides care based on the best available evidence and may engage in research activities within clinical practice. Participates in unit or facility shared leadership. Engages in the education, role development, and orientation of facility personnel, patients, students, families, and visitors. Supports the growth of others through precepting and mentoring when appropriate.

Contributes to societal health outcomes through activities that ensure timely, effective, efficient, equitable, and safe care.

Actively participates in improving national nursing quality indicators and outcomes, which may include involvement in professional organizations.
