About the Position

We are seeking a highly skilled and compassionate Admissions Screener to join our team at Living Legends Health. As an Admissions Screener, you will play a critical role in ensuring that our residents receive the highest level of care and support.

Key Responsibilities
  • Screen and recommend patients for long-term or subacute care
  • Visit designated hospital assignments daily to assess patient needs
  • Coordinate room changes and inter-facility transfers upon request and following approval of resident's physician
  • Interact with residents and family members, co-workers, and clinical and ancillary staff in a non-judgmental, supportive, and calm manner
  • Maintain a daily census record for the assigned facility
  • Perform additional tasks as assigned by management
Requirements
  • Bachelor's Degree or equivalent experience in Social Work, LPN/RN Nursing, Therapy, or another health-related field from an accredited institution
  • Experience in Admissions Screening and long-term care is preferred
  • Must be flexible, professional, and reliable
  • Strong customer service skills
  • Must possess a valid New York State Driver's License and have transportation available for job-related travel
