Job Summary

We are seeking a skilled Transition Coordinator to join our team at Child & Family Services of NY. The successful candidate will provide transition coordination to children and families, ensuring a smooth transition from our care to their community-based services and supports.

Key Responsibilities
  • Develop and implement discharge plans in collaboration with the treatment team
  • Provide linkage to community-based services and supports, including in-home parenting skills training and transportation
  • Assess unmet needs and provide Flex Funds as needed
  • Monitor length of stay and progress toward discharge
  • Participate in weekly programming, treatment, and staff meetings
  • Coordinate with staff and family regarding family progress, child treatment plans, and planning for visits
  • Integrate information from various sources to complete assessments and treatment planning
  • Complete paperwork in a timely manner
  • Presents at treatment team meetings and follows through with recommendations
  • Assess family strengths and unmet needs and participates in developing and coordinating the service plan
  • Involves the family in the development of treatment goals and discharge criteria as well as involving families in decision-making as appropriate
  • Assist the family to maintain their connection and involvement with the child during stay
  • Demonstrate the ability to motivate families to participate in services
  • Provide child skill-building as appropriate with an emphasis on community skills
  • Transport child to visits as needed and appropriate
  • Monitor length of stay of each child and progress toward discharge, and coordinate with the treatment team to remove barriers to treatment progress and discharge
  • Assess need for utilizing Flex Funds as appropriate to facilitate discharge to the child's home community and provide for successful transition
  • Begins discharge planning upon admission and continually assesses the discharge plan and discharge criteria for each resident
  • Makes a referral to the discharge resource within 30 days of the recommendation being made by the treatment team
  • Set up appropriate discharge service upon discharge, and in partnership with family will develop a Continuing Care Plan and a Crisis Plan and follow-up with client, depending upon the need of the family
  • Implements lower-level techniques of therapeutic crisis intervention (TCI) effectively, appropriately, and safely
  • Writes event reports correctly and in a timely manner, including antecedents, the event, and the outcome
  • Participates in TCI training and updates
  • Provide after-care services including home visits to provide in-home parent skills training as needed and appropriate, coordination and linkage to community-based services and supports, continually assess unmet needs and provide Flex Funds as needed and appropriate
Requirements
  • Masters Degree in human services field and two years experience in providing direct services to or in linkage for children with serious emotional disturbance to service, or a Bachelors degree in human services and four years experience
  • Ability to provide professional leadership and work well with an interdisciplinary team of professionals, paraprofessionals, and non-professionals
  • Ability to relate to different social and ethnic groups, effective in developing therapeutic relationships with mentally ill children and their families
  • Experience in managing hostile and aggressive behavior of children
  • Effective communication skills verbally, in writing, and by telephone
  • Must have mobility to access multi-buildings, make home visits, and attend community meetings and visit other parts of the agency
