Position Overview

Title: Rehabilitation Specialist

Key Responsibilities:

  • Formulates a tailored rehabilitation strategy in collaboration with clients, their families, and healthcare providers, ensuring measurable and achievable outcomes.
  • Educates clients about their rights and responsibilities, empowering them to make informed choices regarding their rehabilitation journey.
  • Designs, executes, and modifies a rehabilitation plan that aligns with the client's unique age, condition, and requirements.
  • Facilitates activities outlined in the care plan aimed at promoting the client's independence and enhancing their quality of life.
  • Regularly evaluates the client's needs for therapeutic interventions and assesses the effectiveness of these interventions through detailed reporting.
  • Connects clients with both internal and external community resources to achieve desired outcomes when necessary.
  • Initiates appropriate diagnostic, preventive, and rehabilitative measures to ensure client safety and progress in their rehabilitation plan.
  • Documents and evaluates progress towards established goals, coordinating discharge plans with clients, families, and other healthcare professionals.

Qualifications:

  • Master's or Doctorate degree in Occupational Therapy from an accredited institution.
  • Preferred one year of clinical experience in Occupational Therapy, with a focus on Home Health being advantageous.
  • Possession of a valid Occupational Therapy License in the State of Maine and a current CPR certification.
  • Must have access to a reliable and insured vehicle along with a valid Maine driver's license.
