Clinical Case Manager Lead

We are seeking a highly skilled and compassionate Clinical Case Manager Lead to join our team at Volunteers of America Los Angeles. As a key member of our Clinical Case Management Team, you will be responsible for providing exceptional care and support to our clients, while also supervising and mentoring junior staff.
Key Responsibilities:
- Assign and monitor case loads of team members, ensuring that clients receive the highest level of care and support.
- Conduct comprehensive screenings and assessments with clients to develop a personalized case plan, coordinating and facilitating move-in activities, rental negotiations, and service referrals as needed.
- Monitor case managers on a regular basis to determine the quality and effectiveness of services provided, reviewing cases with the Program Coordinator/Manager and Clinical Case Management Team.
- Assist case managers by identifying and coordinating available services necessary for clients and families to maintain/obtain self-sufficiency and family stabilization.
- Provide training, supervision, and problem-solving guidance for Case Management Team members in close consultation with Clinicians.
- Develop and perform workshops pertaining to program services that meet client needs.
Requirements:
- Master's degree in social work, psychology, or counseling from a fully-accredited college or university.
- Registered with the Board of Behavioral Sciences as an Associate Clinical Social Worker (ASW) or Associate Marriage and Family Therapist (AMFT).
- Minimum of 3 years of complex clinical experience in program(s) serving homelessness, addictions, under-served populations or prison re-entry/diversion.
- Proven leadership qualities.
