Lead Case Manager

We are seeking a passionate and experienced Lead Case Manager to join our team at Beacon Hill. As a key member of our Community Health Program, you will play a vital role in increasing the well-being of individuals impacted by substance use disorder.
Responsibilities:- Meet with participants regularly to assess their needs and link them to essential services, including medical care, shelter, food, clothing, and harm reduction services.
- Develop and implement individualized initial assessments and care plans to support participants' unique needs.
- Educate participants on the range of services available to them, empowering them to make informed decisions about their care.
- Assist participants in entering service programs, ensuring they receive the support they need to achieve their goals.
- High school diploma or equivalent required.
- 1+ year of experience with intensive case management services (may be substituted with an associate's degree in human services or higher).
- Experience working with vulnerable populations.
