Key Responsibilities- Manage the day-to-day activities of specialized programs and/or products, ensuring seamless integration of evidence-based care practices into protocols, policies, and quality improvement initiatives.
- Supervise a team of nurses and healthcare professionals, providing guidance and support to ensure patients meet eligibility requirements and receive appropriate care.
- Collaborate with Health Plans to ensure timely and effective services are provided to patients.
- Develop and implement workflows, protocols, and quality improvement initiatives to enhance patient outcomes and staff performance.
- Act as a liaison between care management teams, ensuring program effectiveness and making recommendations for improvements.
- Participate in internal and external clinical care conferences, promoting staff understanding of tele-management processes and their value for patients, medical providers, and healthcare partners.
- Maintain excellent communication and relationships with home care/hospice teams and Health Plans.
- Assess, educate, and improve patient knowledge of chronic disease, self-care management, and health status changes, including responses and actions through individualized education and multifaceted interventions.
- Review patient records for readmissions, lead root cause analysis, and develop recommendations for quality improvement measures.
- Conduct team audits, analyze trends, and implement corrective measures to address performance or operational issues.
- Assist staff in navigating patient, family, physician, and home care team through education, evaluation, and decision-making.
- Oversee metric reporting, create weekly departmental KPI reports, and assist senior leadership with client outcomes reporting and clinical data analysis.
- Perform all duties inherent in a managerial role, ensuring effective staff training, interviewing candidates, evaluating staff performance, and recommending hiring, promotions, and terminations.
- License and current registration to practice as a Registered Professional Nurse in NYS.
- Valid driver's license (as determined by operational/regional needs).
- Minimum of three years of clinical experience.
- Experience in case management, administration, or discharge planning in a hospital setting (preferred).
- Training in population care coordination (preferred).
- Exceptional customer service skills.
- Demonstrated ability to engage clinical counterparts in collaborative discussions.
- Strong follow-up skills and ability to manage multiple priorities.
- Proficiency in Microsoft Office Suite.
- Knowledge of value-based care models and managed care (preferred).
- Hospice or palliative care experience (preferred).
- Experience as a patient advocate (preferred).
