About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide Professional Nursing Services: Deliver skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care, in accordance with accepted standards of clinical practice.
  • Develop and Implement Care Plans: Utilize a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care, in collaboration with the healthcare team.
  • Communicate Effectively: Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team in a timely manner regarding patient/family needs and status changes.
  • Document Patient Care: Prepare, submit, and maintain documentation as required by the Company and/or facility, including visit/shift notes.
Requirements
  • Education and Licensure: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing, and current nursing license in the practicing state.
  • Experience: Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
  • Certifications: Current CPR certification.
