As a Registered Nurse, you will play a vital role in our care teams, working closely with patients, families, and healthcare professionals to deliver exceptional care and support.

About the Role
  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing personalized care plans to promote optimal health and well-being.
  • Collaborate with interdisciplinary teams to coordinate care, including physical therapists, occupational therapists, and home health aides.
  • Confer with physicians to develop and implement care plans, ensuring seamless communication and coordination of care.
  • Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.

Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related setting.
  • Strong med-surg, ICU, ER, or acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
