About the Role

As a Registered Nurse with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team of skilled healthcare professionals is dedicated to providing comprehensive care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities

  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing personalized care plans to promote optimal health and well-being.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Conduct thorough assessments, develop and implement care plans, and provide ongoing education and support to patients and their families.
  • Monitor patient progress, make adjustments to care plans as needed, and ensure timely documentation of all patient interactions.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.

Requirements

  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related field
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
