About the Role
As a skilled nursing professional, you'll provide comprehensive care to patients with chronic conditions or recovering from injury, illness, or surgery. Your expertise will encompass various aspects of patient care, including:
- Developing and implementing personalized care plans in collaboration with patients, families, and healthcare teams.
- Providing hands-on care, management, and evaluation of patient care plans, ensuring optimal resolution of needs and problems.
- Coordinating care with external providers, including physical therapists, occupational therapists, and home health aides.
- Reporting patient care, condition, and progress to physicians and clinical managers on a continuous basis.
- Discharging patients after consultation with physicians and clinical managers, preparing and completing necessary clinical documentation.
Requirements
To succeed in this role, you'll need:
- Diploma, Associate, or Bachelor's Degree in Nursing.
- Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute settings.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
