Wound Care Specialist

Hospice Buffalo is seeking a skilled Wound, Ostomy, and Continence Nurse to join our team. As a Wound, Ostomy, and Continence Nurse, you will provide high-quality care to our patients with wound, ostomy, and continence issues.
Key Responsibilities:- Collaborate with the attending provider and interdisciplinary team to develop and implement a plan of care for patients with wound, ostomy, and continence issues.
- Provide wound, ostomy, and continence care to patients in accordance with agency policies and procedures.
- Document patient findings and interventions in the electronic medical record.
- Participate in staff education and training to stay current with best practices in wound, ostomy, and continence care.
- Graduate of an accredited school of nursing with current RN licensure in the State of New York.
- Graduate of an accredited program for Wound, Ostomy, and Continence Nursing with certification.
- Valid driver's license and vehicle required for use when on duty.
